What to Bring to Your First Visit
The first time we welcome you to Trail View Dental, please bring the following items with you.
- Goals for treament/cosmetic concerns
- Valid photo ID
- Dental insurance card (if applicable)
- Medical/dental history
- Comprehensive list of prescription medications
- Name of previous provider
We provide insurance estimates based on the info your provider shares with us, though they aren't guaranteed. We’ll file claims as a courtesy, but you’re still responsible for any remaining balance. If the insurer pays more or less than estimated, we’ll adjust your account accordingly. Please ask us about financial policies or payment options before treatment.

Directions
From Highway 41, head toward Grand Island and turn right onto South Locust Street. Continue north until you reach Stolley Park Road, then take a left onto Stolley Park Road. Drive west for about two miles. We’re located just past the intersection with Broadwell Avenue on the left-hand side.
Plenty of free parking is available right in front of our practice for your convenience.
